If you are injured over the weekend: 

If your injury doesn't require immediate attention:

1. Inform site security of the injury - Security will submit the report needed for your case.  

2. Submit a Freshdesk ticket with as much detail as you can provide. 

3. Contact the triage number: (855) 293-4222. They will direct you to an approved medical facility for care. 

4. Modis leadership will contact you for further details the next business day. 


If you are injured during business week:

If your injury doesn't require immediate medical attention:

2. Contact your Engagement Manager (If your site doesn't have an EM, please contact Connie Surich (412-316-0246)) and your Google Manager right away. 

3. Submit a Freshdesk ticket for tracking with as much detail as you can provide.
